The following is a basic set of expectations comprising the patient-physician contract for all patients in my practice. This contract may be modified periodically.
WHAT YOU CAN EXPECT FROM ME
-To provide a respectful, safe, and comfortable environment for patients of all backgrounds, races, religions, sexualities, genders, and beliefs to share any questions or concerns you have regarding your health and wellness. Anything shared during an appointment is confidential unless there are serious concerns regarding the wellness of others, or the wellness of children.
-To be on time and punctual: 15 minute appointments unless otherwise stated/booked. I value your time as well as my own, and will always try my very best to be on time and not keep you waiting.
-To provide thorough and well prepared care. By providing me with the basic reasons for your appointment beforehand, I will be better able to review your chart, prepare any relevant resources, and provide efficient, thorough care.
-To ensure responsible use of medications when needed, and optimization of lifestyle factors to minimize the need for medication whenever possible. My personal philosophy on controlled substances such as opioids, benzodiazepines, and other restricted medications is quite conservative, and I try to avoid using them unless absolutely necessary
-To order bloodwork, imaging, and other tests only when necessary. You will be contacted with any abnormal results, but not contacted with normal results, unless otherwise stated.
WHAT I EXPECT FROM YOU
-To treat physicians and staff with respect: zero tolerance for abuse of staff. We reserve the right to dismiss any patient who is verbally or otherwise abusive towards staff
-To be on time for your appointments, or to call ahead as soon as possible to rebook or cancel. Life happens and sometimes there may be things outside of your control that may affect your ability to attend an appointment. We would ask that you inform us as soon as possible if you will be late or unable to attend your appointment. We reserve the right to charge a "no-show" fee and/or dismiss any patient with a pattern of tardiness or not showing up to appointments.
-To provide information regarding the purpose of your appointment prior to the appointment to allow for adequate preparation and appropriate time to be booked for the appointment, and to avoid an excessive number of issues to be addressed per visit. Trying to address too many issues at one visit can make it more difficult to be thorough, and to stay on time for the next appointments. Typically, 1-2 issues can be properly addressed during one visit. Appointments should also be kept to 1 person per appointment, unless other arrangements have been made prior to the appointment.
-To follow instructions regarding the safe use of medications, and to sign an opioid contract should you require prolonged therapy with an opioid or other restricted medication
-To complete any testing that is agreed upon during an appointment, and attend any specialist appointments you have been referred to.
-Repeated violation of the above may lead to further discussions and possible dismissal from the clinic
